Service Description

Psycho-educational evaluations are evaluations that are completed to determine whether your child should be receiving special education supports and services under Article 7, which is special education law in the state of Indiana. As a result of the assessments that are completed with your child, a full evaluation report with recommendations will be provided to you. Included with this prices is the psychologist advocating for those recommended services for your child in the school. The 13 eligibility categories defined in Article 7 include: 1. Autism Spectrum Disorder (ASD) 2. Blind or low vision (BLV) 3. Intellectual Disability (ID) 4. Deaf or hard of hearing (DHH) 5. Deaf-blind 6. Developmental Delay (DD) 7. Emotional Disability (ED) 8. Language or Speech Impairment (LI or SI) 9. Multiple Disabilities (MD) 10. Other Health Impairment (OHI) 11. Orthopedic Impairment (OI) 12. Specific Learning Disability (SLD) 13. Traumatic Brain Injury (TBI) Any of the following services can be added to your base evaluation for an additional price: - Autism Evaluation (add $1050): Includes the Autism Diagnostic Observation System, Second Edition (ADOS-2), Social language evaluation from the Speech-Language Pathologist, Occupational Therapy Evaluation to address any fine motor or sensory needs, and an evaluation from a Board Certified Behavior Analyst (BCBA) for behavior if necessary. - ADHD Screening (add $750): The Test of Variables Of Attention (TOVA) is a computer administered continuous performance test which has become widely used as an adjunct for detecting characteristics of ADHD. It is also an objective method of assessing characteristics of ADHD, without relying solely on parent’s/teachers reports and/or an interview. We offer this test as a diagnostic tool to determine whether your child has characteristics that are similar to that of a student who has been diagnosed with ADHD. - Behavior Support (add $600): A Board Certified Behavior Analyst (BCBA) will complete a Functional Behavior Assessment (FBA) and Behavior Intervention Plan (BIP). - Speech & Language Evaluation (add $550): Includes articulation and/or receptive, expressive, and social language concerns assessed by a Speech Language Pathologist. - Occupational Therapy Evaluation (add $500): Includes fine motor, sensory, and/or visual motor concerns assessed by an Occupational Therapist.
