How to Request an Educational Evaluation from Your Child's School

Did you know that as a parent in Indiana, you have the legal right to request a free educational evaluation for your child through your public school? Many families don't know this — or don't know how to start. Here's exactly what to do as the new school year begins.

Start with a Written Request

Your request must be in writing. A verbal conversation with a teacher doesn't start the legal clock. An email or letter does. Your written request should include:

• Your child's full name and date of birth

• The school they attend and their current grade

• A brief description of your concerns — academic, behavioral, attention-related, or all of the above

• A clear statement that you are requesting a special education evaluation under IDEA

• The date of your request

Send it to the school principal or special education coordinator and keep a copy for your records.

What Happens Next?

In Indiana, once the school receives your written request, they have 10 school days to respond with either a consent form to begin the evaluation or a written explanation of why they are declining. Once you sign consent, the school has 50 calendar days to complete the full evaluation.

• Request mediation through the Indiana Department of Education (IDOE)
